I assume that "Have been seen by player" means that the player must have >x %object% - and that "Be visible to player" meant that the %object% must be in plain view.
Is this correct?

If it is, how come that my player are still able to <Read inscription on a tombstone without having >x tombstone first?
What am I doing wrong here?

I think the meaning of "has seen" is closer to "was in same as".

What am I doing wrong here?

"Has been seen by player" just means that the player has been in the same location as the object. You would have to set a variable in an X TOMBSTONE task and have a restriction in your READ INSCRIPTION task where if the variable is 0 then you would get the response "What inscription?"

"Be visible to player" is not, in my opinion, defined properly in ADRIFT. An object can be inside another object (and thus not actually "visible" to the player) but if you X this object you will get the full description.
